His name was Derek Brien, the place he crashed was one of the residential roads with no runoff taken at over 150mph.

Very much more likely, and usually more than 1 per year.

Even this century, I think there's been at least one fatality every year except 2008(?). Obviously there weren't any in 2001 but that doesn't count.
