Have you also heard about the FIA will penalize in future Qualifyns, when you do not get your car back to the pits, like lewis did??

Yeah, taking someone out on purpose and pushing a car back to the pits when he shouldnt are the same thing. Makes sense.

Petrov joins Badoer and Hakkinen who got two penalties in one race. I believe there is still someone in this group, I remember a driver had a jump start then he went to quickly in the pitlane when he took his penatly so he got another one. It happened in the late 90's, perhaps on a British GP, but I don't find any info about that.

I can recall
Massa in Magny-Cours 2002: jump start and crossing pit exit line
Heidfeld speeded twice in Montreal 2002
Martini and Katayama in Montreal 1995: jump start for both, blocking for Martini and pit lane speeding for Katayama
