I like how nothing happened in this race then that huge brake failure/t bone/whatever it was happened the second indycar went green and had a big flipping big one simultaniously

Okay just look at these two screenshots. One from earlier in the race, one from after the incident that brought out the SC. See that tree hanging over the barrier? It wasn't there before. I'm going to argue a car's gone into the forest, and that's why the race was red flagged. No word on it until now, hope that driver is okay..

That would be Dalton. Both he and Pombo were taken to hospital.

My guess would be Dalton lost brakes or had a stuck throttle, clipping Adelson at some point.

It was Matt Dalton who flipped at COTA as well. That's a rough couple of races.
